Customized Forms

ZipChart utilizes the forms that are currently used in your office, digitizes them and makes them available for use electronically. These forms can be written on using either the Tablet PC stylus or typed on at a workstation.

Customized forms can be created from any forms that are already used in your office (Lesion sheets, Mohs surgical reports, Consent forms, etc.).

When you use these forms, many fields will be automatically populated from the current values in the ZipChart database (i.e. name, DOB, and date of service).
